The following support materials are required for the lead applicant only.
- IRS 501(c)(3) designation letter (for the Lead Applicant or Fiscal Agent)
- Production history for ensemble theatre for the previous three years (for the Lead Applicant or Fiscal Agent)
- Bios for ensemble theatre artists and key personnel
- Work Sample(s)
- Artists/theatre companies should submit videotapes or DVDs, scripts, and other relevant materials that the artists feel best represent their work. Videos or DVDs are not required for those who, because of Equity restrictions, cannot document their work in this way.
- Applicants should cue videotape or indicate where to begin watching the DVD. In general, panelists will watch no more than 2-3 minutes of a tape if at all.
